The story unfolds in a mysterious hotel on the border between Pennsylvania and West Virginia, serving as a trap for America's most dangerous serial killers. The protagonist, Justine, a rookie U.S. Marshal, finds herself trapped in this twisted setting, harboring within her the alter ego Dr. Carnival, an ancient monster and murderer.

The game offers intense action with battles against seven serial killers, where each player death creates a ghost of their former self that fights alongside them in subsequent attempts. The use of enemies' blood enhances Justine's abilities, adding strategic depth to the gameplay. Additionally, the game supports online co-op multiplayer for up to four players, allowing for cooperative play with friends.
